Sorry for the long post, but I wanted to show you how a ban email looks like informing about Account Closure.
If you read the wording carefully you can see it says that there were repeated violations.
So I assume Linden Lab doesn't act on the first report, they probably keep a archive of reports before they put a account on hold
Oh and fun fact they get to keep your Lindens also when you get banned.

What is Linden Lab's policy on disturbing the peace?
Every Resident has a right to live their Second Life. Under our Community Standards policy, interfering with that right by griefing, assaulting, spamming, caging, or otherwise impeding another Resident's enjoyment of the Second Life experience may be considered disturbing the peace. When detected, individuals and groups engaging in harassment may be subject to escalating sanctions, ultimately resulting in termination of accounts, closure of groups, removal of content, and loss of land or access to land if the behavior does not cease.
There are three categories of disturbing the peace which are breaches of the Community Standards:
(1) Visually impairment of another resident's experience through the use of scripted objects, visual spam, graphics crashers, deformers, etc.
(2) Preventing textual communications via unsolicited advertising content or spam
(3) Intentional generation of lag, sim crashes, server impedance, decreased computer performance, etc.
Unfortunately, an account owned by you has been determined to have been used to disturb the peace, and, despite repeated sanctions, or because of severe violations, you therefore may no longer engage in any aspect of the Second Life service.
This concludes our investigation of your appeal. You may not return to Second Life on any account, new or existing. Please consider the matter resolved, as no further communications will be sent.
